First thing to evaluate in such case is the change in blood pressure values at rest, during or after exercising, laying and standing.
If significant change in blood pressure values ( 20 mmHg or more ) in different activities, evaluation by your Cardiologist and adjustments in your treatment are necessary.
Another condition to consider is heart and its valves function, cardiac or valves insufficiency may result in such symptoms.
In both cases, Cardiologist evaluation is necessary.
