What Causes Constant Seizures While On Keppra?

Question: I recently changed from Dilantin 300mg/day to Keppra, beginning with 1000mg/day and then with the understanding that after 3/4 weeks if necessary I would increase my dosage. For the first week it was like magic I had NO seizures, and then exactly a week later everything fell apart. I am having seizures constantly especially in the afternoons, by night time I have to load up on Valium to get to sleep. My neurologist has gone away on a. 3 week vacation and I don't know what to do! I've raised my Keppra dose to 2000mg, should I go back on the Dilantin too and use them both? My life is hell right now. I can't go anywhere or do anything and I am so tired. Please help.

Hello,
I have gone through your question and understand your concerns.
Keppra is a good antiepileptics. The dose can be increased to 1500 or 2000 mg daily dosage.
Additionally clobazam can be added 10 mg once or twice daily.
Ideally dilantin should not be stopped at one go and better to reduce dose gradually before stopping.
If the seizure control is not appropriate then either Valproate, phenytoin or lacosamide may be added.
What is the diagnosis of seizure?
Any brain lesions?
Any family history?
Any myoclonic jerks or absences?
You should provide more details.
