Suggest Treatment For Perianal Hematoma

default
Posted on Thu, 20 Nov 2014
Question: Hi there,
I have what I think is a perianal hematoma. I have had this condition twice in the past. The first and most current time (yesterday) this was treated by lancing the skin and removing the clotted blood from the area. The second occasion was not as bad and went away after some time.
As mentioned, the doctor lanced the skin and removed a substantial amount of darkish blood
from the area yesterday. I have since had 2 bowel movements which were painful and after the second movement (only 15 minutes prior to writing to you) I have realised while bathing that the the lump still seems quite large. I am not worried that this is still quite painful, as from the previous experience I expected to have some discomfort for up to around 4 to 5 days, however, the fact that the lump is still present is quite worrying.
Please advise if this is normal or whether I need to revisit the gp.
thanks so much. XXXXXXX

Brief Answer:
Re-accumulation / Pack inside the wound

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.

It says the lump in the first incidence went away with medicines.
The present one was lanced yesterday, had a substantial amount of darkish blood. Was not there any pus or so ?

Since it is the first post-operative day, it is expected to have lots of edema. It takes a few days for the edema to be reduced even after lancing. I think there may be a packing gauze inside the cavity which is usually kept to stop bleeding. This may also lead to large appearance. Please confirm whether there is a pack inside. If there is no pack , it is wise to re-visit the GP to see for the reason of
large lump in spite of surgery and He would confirm the reason and may solve the problem you have. You also have a significant pain as per your history.

Take the proper dosage of the medicines you must be taking.

I hope this answer helps you, please feel free to ask for more if you need to.

hi,
there is no packing.
there was no puss to my knowledge. There appeared to be only a large clot of darkish blood.
Is the swelling due to edema in your opinion? should I wait several days to see if the swelling subsides or is it wise to see the gp as soon as possible?

Brief Answer:
Please see the GP ASAP

Detailed Answer:
Thanks for your feedback.

It is wise to see the GP as soon as possible (ASAP) as the swelling after hematoma drainage should be minimum and the pain too should be tolerable.

You should not wait to see if the swelling subsides.
