Question: Hi, I would like to switch from Seretide to Alvesco, I have been having trouble controlling my Asthma lately. My peak flows are strong, I have had my heart checked, and a thorough physical. I think the issue lies in the small airways and am concerned that Seretide isn't as effective as other drugs for reaching the small airways. Also is there an equivalent of Ventolin that reaches the small airways?

Brief Answer:
Technique and method of MDI is very important.

Detailed Answer:
Thanks for your question on Health Care Magic.
I can understand your concern.
Best treatment for asthma is inhaled bronchodilators and inhaled corticosteroid (ICS).
And technique, method, the way patient is taking MDI (meter dose inhaler) is very important.
With faulty technique, drug deposition in lungs is not achieved and hence proper therapeutic benefit can not be gained.
Seretide is having salmeterol (long acting beta 2 agonist - LABA) and fluticasone (Inhaled corticosteroid - ICS) .
Alvesco is having only ciclesonide (ICS).
So seretide is better than Alvesco.
So in my opinion, you should continue with seretide only. Proper technique of taking MDI is very important.
All MDI s reach to small airways when technique of taking MDI is proper.
Ventorlin is having levosalbutamol. Salmeterol is far better than levosalbutamol.
So continue seretide MDI with proper technique. It will definitely reach to your small airways.
Please let me know
1. Are you taking MDI properly?
2. How many times in a day you take seretide?
Please reply me answers of above asked questions, so that I can guide you better.
I will be happy to help you further. Wish you good health. Thanks.

Thanks very much Doctor, it turns out my inhalation technique was incorrect, thus i have invested in a spacer. Are there any other suitable alternatives to Seretide for allergis asthma?

Seretide is having good combination. It i having Salmeterol and fluticasone.
Both are long acting and considered as first line of therapy. So continue with Seretide.
Along with Seretide ,you should also take oral combination of anti histamine (Levocetrizine) and anti allergic ( Montelukast). This will also beneficial in your case.
