Question: My doctor has me switching from 25 mg of Pristiq to 10 mg of Lexapro. I would like to hear how I should make the transition from one medication to the next medication. Thank you

I can understand your concerns.
Practically, even if pristiq 25mg and lexapro are taken together, there are no serious drug interactions. I use the two drugs together quite often in my clinical practice and have not seen any serious drug interactions.

Even then, if you want to go slowly, I would suggest you to reduce the dose of pristiq to half and simultaneously start with lexapro 10mg. Continue this for 1 week and the pristiq can be stopped then. Please keep your doctor informed about this.
