My 92 Year Old Mother Takes Carvedilol For An Irregular

Question: My 92 year old Mother takes carvedilol for an irregular heartbeat. She is currently taking 6.25 mg per day it has worked well until the past week when I have taken her blood pressure, I noticed the irregular heartbeat icon on three different occasions. When I read her prescription, I noticed that her dosage is 6.25 mg 2@ per day. She used to take 24mg but they found it was dropping her heart rate too low. What d should I do? and is this dangerous? She does not exhibit any symptoms and she says she feels good. I am scared.

I have gone through your query and here is my opinion.

If she is asymptomatic, I don't think that she needs any intervention.

An irregular heartbeat, which is benign and not causing any symptoms or hemodynamic deterioration (like fall in blood pressure) is not a matter of great concern.
Irregular heartbeats called VPBs (Ventricular Premature Beats) are quite common in elderly patients.

Such patients should be under regular cardiologist follow up and should periodically undergo 2D echo to ascertain the pumping function of the heart and to detect any deterioration.

As per protocol, patients are always maintained on lower doses of it if higher doses are not tolerated by the patient.

The same is being done by her clinician and there is nothing to be worried about it.

This morning I took three blood pressure readings. The first one showed the irregular heartbeat symbol. One minute later I took another reading and the irregular heartbeat symbol was gone and I took a third reading one minute later and it didn't show an irregular heartbeat. Blood pressure was good also.

As we have discussed that an irregular heart beat which is benign,not causing any symptoms or haemodynamic deterioration is usually left as it is and is not a big reason to worry.
Such patients are usually maintained on lowest dose of anti arrhythmic drugs(such as Carvedilol),as being done in your mother's case.

Carvedilol belongs to a group called alpha and beta blockers and act by blocking the action of neurochemical epinephrine on the heart and blood vessels.
This blockade lowers the heart rate,controls irregular heart beats,decreases blood pressure and decreases the strain on the heart.

If her blood pressure is good and she is not having any symptoms,please don't be worried about it.
