Question: MY 62 YEAR OLD HUSBAND FALLS ASLEEP ALMOST EVERY TIME HE SITS FOR ANY LENGTH OF TIME - AT HOME, AT WORK, AT CHURCH, WHILE VISITING HIS VERY BELOVED GRANDCHILDREN! HE IS GETTING VERY WORRIED ABOUT KEEPING HIS JOB, ACTUALLY. HE HAS DIABETES (TYPE 2) AND FACTOR 5 LEIDEN. GOT ANY SUGGESTIONS TO WHAT IS GOING ON? WE ARE DOING OUR BEST FOR HIM TO GET MORE SLEEP AT NIGHT, BUT HE HAS BEEN GETTING 5-6 HOURS FOR A LONG TIME. THIS PAST WEEK HE'S BEEN GETTING AT LEAST 8 HOURS, BUT HE IS STILL SLEEPING. HE ALSO USES A CPAP REGULARLY.

I read your query and understand your concerns.

From the available description it appears to me that he is probably suffering with obstructive sleep apnea which is common among those who are slightly overweight or have loose tissue in the airway due to any reason. Considering the fact that he uses CPAP I do not think we need to anything about sleep apnea but using stimulants during day can be a answer to his problem. The options include modafinil and armodafinil. I often use 100-220 mg of modafinil or 50-150 mg of armodafinil in such individuals with appropriate outcome.

Giving more time to sleep is helpful but does not yield proper results as there are probably multiple breaks during sleep and patient remains unaware of these breaks on most occasions.

In case you feel there is no sleep apnea I feel ordering a polysomnography can reveal the problem faced by your husband.
