Well, there are many reasons that can cause pain in the left side of the chest. According to your age of 87 years old, and the fact that you have performed pericardectomy 18 years ago, I can only assume that this is anginal pain.

Now you must be knowing that Angina is pain that is elicited from blockages within coronary arteries (arteries that surround and feed the heart with oxygen and valuable nutrients) - starts on exertion or resting state, in the mid chest or left side of chest and radiates to the jaw, left shoulder and left arm. It goes away with discontinuation of exertion or use of sublingual (under-the-tongue) nitrate derivatives.

Since my main concern is that this is angina, I would recommend you first to run some tests regarding the issue:
- an EKG - which assesses the heart's electrical and conduction systems, at resting state; and
- an echo cardiogram - which allows us to evaluate the structure, motility, size and function of heart chambers and valves.

It is only after these tests are carried out, when we can further properly assess the situation and make decisions regarding further tests, treatment strategies and medication.
