Question: hi. a 58 yr old man has a black small about 3cm spot on ball of foot. Has been complaining of pain.

Read and reviewed your query in context to your health concerns about painful black spot on ball of the foot.

My Impression and comments-
From the submitted facts,You seem to suffer from-Plantar Warts with a black spot/ or it could be-friction blister with a tight shoe.
Consults with Surgeon / and podiatrist-to rule out other causes-like-Callus/Foreign body Wart or blister/ with tight shoes and defective plantar arch, metarsalgia/sesmoiditis from sesmoid bones on ball of foot/ flat foot or pes planus.

Remedy-
ICING and cold compresses-to resolve it in a day or two.
Tab Advil-Ibuprofen-400 mg 1 x 2 per day x 3-5 days

If the spot doesnt' resolve-Consults with Surgeon/ or podiatrist,who would check physically it and for its causes in the shoes and in the plantar arch.

X-ray of the foot and physical checks would fix the cause and would be treated accordingly by the podiatrician or Surgeon.
Callosities / FB wart or granuloma/Plantar warts -would need Surgical Excision by a Surgeon.
Flat foot would need corrective tailmade shoes.
Cushioned loose shoes would be needed if the small shoes is the cause.

Next appointment-
Working on the lines suggested would take 2-3 weeks.So keep Me updated, for review and further care if needed.
