Suggest Treatment For Intense Stomach Cramps In The Middle And Lower Abdomen

Question: I was awakened at 2 am. With intense stomach cramps . It is in the middle to lower stomach and comes every three minutes or so.
It feels like I need to release gas but when I do it doesn’t help. I’ve taken Mylanya, Pepto bismol and 2 gas pills but it’s still not better. It’s been going on for about 2 hours , when the pain spasm comes it’s very strong.

Hi,

From the history, this looks to be enteritis with associated symptoms as you have mentioned. Since the medications that you have taken are not helping, it is possible that the problem can be of more gravity and needs a proper clinical evaluation and investigations.

I would advise you the following in such a situation:
Also take an antispasmodic medicine like Dicyclomin or so.
Take a PPI like Omeprazole.

If this does not help then obviously you need to visit ER for clinical evaluation, examination and investigations like:
X-ray of abdomen in standing position to see for any intestinal obstruction.

Tests of stool, blood and urine. Ultrasonography of whole abdomen. You may need additional antibiotic active against intestinal infection if there is any evidence. You also need a proper antispasmodic medications for pain relief immediately.

The pain comes in waves of intense pain followed by a few minutes of relief. Could it be gall or kidney stones?

Such pain that comes and goes is called colic/spasmodic pain and can occur in intestinal or due to stone in the ureter usually. Gall bladder colic will be in the right upper region.
