I Have Been Having Weird Symptoms For The Last Three

Question: I have been having weird symptoms for the last three years. It started out with ringing in the left ear and facial weakness on the left side along with numbness in tingling. I have had a few MRIs that show hyperintensities which I know are not lesions. I continue to have odd symptoms repetitively. Over the last three years I have become more clumsy, I trip, and I drop things very often. I have also been experiencing severe chronic fatigue that never seems to go away no matter how long I sleep. Within the last eight months my leg my left leg has started to go numb with feelings of pins and needles shooting up my leg and my toes feel numb. Over the course of the last few weeks I have started having weakness in my right leg this is new for me because I’ve never had any right sided symptoms. I have had an ANA which was negative I’ve had my vitamin D and B12 checked which were also normal my thyroid labs have all been normal. My symptoms continue to progress and make it very debilitating to continue my day-to-day tasks at work and in my personal life.

I also need to mention that I have had MRIs of my entire spine with no presentation of lesions. My spine is in good condition and I do not have any bulging discs or herniations.

I will be seeing my new neurologist in a few weeks I’m nervous because I’d feel like I’m probably not going to get answers again. The last neurologist I saw told me that due to my weight that that’s probably why am having issues I am only 200 pounds, said I have migraines and sent me away.

My symptoms just continue. I really need to know what questions to ask at my appointment to get the most out of it.

My primary has mentioned MS multiple times, but says she cannot make that final diagnosis.

Any input would appreciated

Brief Answer:
I think that MS should be considered and evaluated further.

Detailed Answer:
Hello and welcome to 'Ask a Doctor' service.

I have read your query and here is my advice.

I agree with the primary Doctor's opinion that multiple sclerosis should be considered as a possible diagnosis and should be investigated further since your symptoms continue, are repetitive and are affecting different parts of the body.

You should ask your Neurologist when you see him about his opinion regarding these symptoms and MS, and for further necessary examinations in order to achieve a correct diagnosis.

The MS diagnosis is achieved by symptoms, imaging studies ( MRI ) and spinal tap with subsequent examination for oligoclonal bands.

Evoked potentials also may be of help.

If MS is ruled out after such evaluation, stress may play an important role in your symptoms.

Hope you found the answer helpful.

Let me know if I can assist you further.

Do you feel patients can present with MS even without true lesions?

I have the spots but nothing classified as a lesion at this point.

Brief Answer:
It is possible.

Detailed Answer:
Hello again.

There is no single test to diagnose the multiple sclerosis.

There may be no lesions in the MRI of the brain and spine, and the patient may be diagnosed with MS after clinical evaluation, exclusion of other possible conditions and other tests results.

Hope I helped you.

Feel free to discuss with me if you have further questions.
