Question: I have an elevated sed rate and elevated s reative protein with a negative ANA. My joints are very painful especially my shoulders and my hips. My dr thinks its drug induced myopathy from my abilify. I'm not so sure. I've been on the medication for 6 years. Looking for a second opinion.

ANA is one marker for autoimmune diseases particularly lupus but there are other conditions that can cause joint pain such as rheumatoid arthritis, scleroderma, polymyalgia rheumatica or a hemoglobinapathy eg sickle cell or other abnormal hemoglobins.

Hemoglobin electrophoresis blood test would determine the type of hemoglobin present to make sure that there are no abnormalities.

Rheumatoid factor and anti cyclic citrullinated peptide antibody are used help in the diagnosis of rheumatoid arthritis.

The ESR may be normal in persons with scleroderma so this is less likely though it is a cause of joint pain. It is diagnosed mainly based on signs and symptoms presented, the appearance of the person.

Polymyalgia rheumatica- characterised by acute/sudden onset of pain in the shoulders, hips, torso (back and pelvis) and neck- worse on rising in the morning.
There are no particular blood tests for this condition, it is diagnosed mainly based on the recent severe onset of pain affecting daily living.

You can consider seeing a rheumatologist for assessment. He or she can take a history and examine you and determine if further testing is needed.
Testing may include blood tests and imaging such as x ray.
