Question: I have an condition i would love some input please. I have not been given a specific to which or both but I am dealing with SI nerve pain or piriformis dysfunction. Though I have several areas of discomfort due to body trying to compensate the key area of moderate to severe pain is a spot just below the illium on my right side close to the back of the butt. I am trying to figure out the cause and have one question to you in regards to causal reason. I have a lower lumbar arthritis and have read that lower lumbar can be a cause, but if it is a cause, what location does lower spine affect compared to a sacrum problem.

I have gone through your query and here is my opinion.

In piriformis syndrome (where there is irritation and compression of sciatic nerve by piriformis muscle),there can be sharp, severe,radiating pain from the lower back or buttock,down the back of the leg and in the thigh,calf, and foot.
The pain is intermittent and the site with maximum pain intensity may vary.
As you have lumber arthritis also,the symptoms may get amalgamated.

The diagnosis of Piriformis syndrome is just clinical (and so difficult too) based on the patient's symptoms,history and physical examination.
There is no specific blood test,muscle biopsy, or imaging test to confirm it.

Symptoms of it may mimic the diseases of spine,inter vertebral disc,bursa,joints,nerves elc and symptoms of piriformis syndrome can be present along with these other conditions such as sacroilitis,lumber arthritis and hip bursitis.

CT, MRI,(EMG)electromyography,Nerve conduction tests,Vitamin B12,Vitamin D,S.Calcium,Thyroid and parathyroid levels and injection tests may help your clinician to evaluate you further.

Treatment of piriformis syndrome is physical therapy,exercise,gait adjustment,stretching,application of heat, NSAIDs, opiods and local injections(local anaesthetics and corticosteroids).
