Question: I have a patient- a 32 year old white female, divorced 2 small children diagnosed with multiple system atrophy which has given her “terrible” excruciating pain from which she can not find relief. She is bedbound but can take a few staggaring steps to the bathroom. The pain is in her head as well as the rest of her body. With this head pain- is it the correct diagnosis - she did have the cross bun sign on her MRI. THC:CBD not much help as narcotics are not as well. Anything new that you know of?

Brief Answer:
Opioids can be last chance

Detailed Answer:
Hello dear welcome to Ask a doctor's service.
I read your query and here is my advice.

I am sorry to hear this situation of pain about her. The most powerful pain management medications we use are tramadol and morphine but they develop opioid tolerance so you may need to increase the dosage of them.
So opioids can be last chance for pain management in her.

Hope I have answered the question.
Let me know if I can assist you further.
