Brief Answer:
can use alternative muscle relaxants...

Detailed Answer:
Hello,

I have gone through your query and here is my advice.

As per your history, you seemed to have already diagnosed with SPMS and conditions are getting worsen.

So for further assessment, I would suggest you consult with a Neuro physician for the detailed clinical examination which may help to know about the present clinical staging of SPMS.

For stiff and painful muscles, you can use alternative muscle relaxant Chlorzoxazone or Thiocolchiside instead of tablet Baclofen and Diazepam after physician consultation.

Along with that, continue physical therapy and regular exercise after physiotherapist consultation which will help to improve stiffness.

You can apply diclofenac gel locally. Heating pads will also help. Do regular follow up with the physician. Take a balanced diet.

Hope I have answered your query...
Let me know if I can assist you further.
