How Complicated Is The Molar Tooth Implant Operation?

Question: I had asked a question few days back and details are as below. As requested the X-Ray and photos are attached for your reference. Kindly advise if we are at right path with the treatment plan. I have added comments on the X-Ray image itself on the doctor recommendation without using braces.

My wife lost couple of her bottom molar tooth few years back and we are planning for an implant now. We visited few doctors and they have different opinion on the treatment.

Current condition is that her upper tooth have dropped down a bit which need to be fixed before placing an implant.

1. One of the opinion is to put the braces and move it up which will take few months and then place the implant.

2. Another opinion is that one of her upper tooth is not down to fix it through braces. Instead they trim it up and put a cap. Another tooth needs root canal and then trim it and put the cap. This might take XXXXXXX 3 months to complete entire process.



Question:

1. Which of the above 2 solution is good?

2. How complicated is the implant operation? How much time it takes for healing

Hi,

Well, it depends on condition of the upper tooth.

It appears to be supra-erupted that is it has come down due to lack of opposing pressure from lower tooth as it is missing.

So first of all please send me an x ray of her upper tooth.
Also if possible click a picture of her upper tooth and upload it here.
Please let me know that is her upper tooth firm enough or is it appearing loose on moving it with finger?

Trimming and capping is better option, if time is a concern.

Detailed Answer:

Hi,

I have viewed your wife's pictures and x rays and what I can see is her roots of teeth that have moved downwards still have sufficient bone support.

So you can go for root canal treatment of both these teeth followed by trimming and capping.

Then the lower implants can be placed.

It is a treatment which will take less time so it is a better option if time is a concern.

But in case if you can wait, then putting braces to pull the teeth up is more appropriate as in that case your upper teeth will not need any trimming and capping and the natural tooth structure can be saved.

So you can decide accordingly.

If you would ask my genuine advise you should always opt for saving the natural tooth structure as much as possible.

Please let me know your priority first.

Thanks for the response.

We thought the trimming option is good option for us as we are planning to travel abroad in next few month.

Two things top consider here.

1. For the extreme end implant, the both the doctors told they can't place the brace. Either removing the upper teeth and placing implant both lower and upper or trimming upper tooth with root canal is the only option.

2. However, for the second implant, both the doctors had different opinion.
a. One of the doctor told to move the upper tooth with braces and then place the implant at bottom.
b. Other doctor suggested trimming without root canal as it has not come down too much. If I remember properly he told 0.2 mm he will trim and then place cap.

Not very sure what to do. Do we have any long term side effects if we trim the tooth?

Hi.
You can then go for trimming and capping of the teeth.

If done properly there is a good response of trimming and capping of the teeth too.

As far as side effect is concerned, trimming natural tooth and replacing with metal is itself loosing the natural structure, but if it is a must to do then getting it done by an expert hand will not cause any side effects.

So u can proceed.

Couple of more questions please.

1. Do you also agree that the extreme top tooth can not be fixed with braces and root canal trimming is the only option?
2. For the other tooth, do you advise trimming without root canal? Is there a possibility of developing sensitivity if we do so?

Trimming should be done with root canal.

Hi.
See without a clinical measurement it is difficult to say that the tooth can be pulled up with brace or not.
But yes it needs considerable pulling, so root canal followed by trimming and capping can help in getting it fixed soon.
Other tooth that is advised for trimming without root canal treatment, but still you can go for intentional Root canal treatment to avoid chances of sensitivity and pain post treatment..
