I Am A 74 Year Old Male Who Had A
Question: I am a 74 year old male who had a total prostatectomy in September of 2005 performed by a top urologist using the Davinci Robotic machine. My Gleeson scale was 7 and the cancer was found in three parts of my prostate and was an aggressive form. Since then, my psa was constantly less than .01 for over 12 years. In may of 2018, 12 years and 8 months since my surgery, a PSA test showed a reading of less than .02 Another test in January of 2019 was .02 and a recent test in February was less than 0.1. Based on these readings, do you think my cancer has recurred? Should I be concerned?

You have done well after prostatectomy. PSA has mostly been below detectable levels. The recent jump from 0.02 to 0.1 in 1 month seems biologically implausible. We need to observe the trend a bit, maybe get another level after 2-3 months.
But we should remember that the definition of biochemical recurrence is 0.2 or more, sustained over time. So even with this report I wouldn't worry much.
