Question: I am a 35 year old female 5'5" 55kg mixed diet no addictions underwent medical termination of pregnancy with pills. Suffering from severe acne since pregnancy was detected and now regular menses started but acne increased. Papulo pustular eruptions on forehead and cheeks leaving behind post inflammatory hyperpigmentation. Tried retino A local application but erythema and acne not subsiding.new eruptions daily. Kindly suggest topical and /or oral treatment and products for skin care. Currently consuming iron and b12 supplements on a regular basis.

For inflammatory acne I would suggest you to take oral Doxycycline 100mg, twice daily for 6- 12 weeks under guidance of doctor. In addition I would suggest you to apply topical benzoyl peroxide gel e.g Microbenz 3.5% gel on acne for day time use. At night you may use a combination gel of Adapalene 0.1% and 1% Clindamycin e.g Deriva CMS gel. Apart from that I would suggest you to use a gentle cleanser e.g Cetaphil gentle cleansing lotion, twice daily, before you apply topical medications.

Dr can retino A be used as a night cream on a regular basis? What are the precautions I would need to take? Is there any other good tretinoin night cream? Also can you please suggest a sunscreen to be used for daily basis.. I use MAC foundation daily to conceal acne... would that worsen my acne? Also is there any change I need to make in my diet?

Hi.
Instrad of Retino, I suggest you to use Adapalene and Clindamycin combination gel e.g Deriva CMS gel at night. Once you acne resolve, you may then use Retino for maintenance of improvement. You must use a non comedogenic sunscreen like Noray sunscreen or Photostable Gold. Avoid using cosmetics, even a sunscreen, if possible. Just use the topical medicines along with oral antiacne medicines.There are no strict dietary precautions or recommendations except avoiding high calorie diet like sugary drinks, too many sweets etc.
