Question: I am 44 and was diagnosed with type 2 diabetes about a year ago. I have been on metformin... 1000mg in morning and 1000mg in evening. The last couple weeks, I started feeling nausea and shakiness...I started taking my blood sugar more frequently, and what I discovered is a consistent fasting blood sugar of 175, and it spikes to 300 after a meal. I experience shakiness and a feeling of hunger, even when my sugar in high (200ish) When I exercise, I can bring it down, but it spikes back to 175 even without eating. I haven't gotten a call back from my doctor in last couple days, and I need his referral to get in with a endocrinologist. 1) What do you suggest? 2) Should I go to a walk in clinic? 3) How long does my sugar need to be high before I have irreparable damage? Thank you.

Hello, I'm Dr. Branch, thanks for using Ask a Doctor. While your blood sugars are high and need to be lowered, they are not so high that I would say it is urgent that you been seen immediately. It would take at least months to cause any damage with blood sugars at this level, so I would not be too concerned about them in the short-term. Making an appointment to see your doctor (you would not necessarily need to see an endocrinologist for this) in the next week or so should be fine to have your medications adjusted and probably start another medication in addition to your Metformin. Your diet may also need to be looked at as another possible factor in the higher blood sugars.

As for the symptoms you are experiencing, it is unclear to me whether they are related to your blood sugars or not, normally patients would not have any symptoms with these blood sugars. So it might help to see how you feel after getting the blood sugars under better control, and do further work-up at that time if they persist.
