I Am 31 Years Old. I Had A Laprasopic Myomectomy

Question: I am 31 years old. I had a laprasopic myomectomy in 2015 for 3 subserosal fibroids: Anterior wall 2cm, Fundus 5cm, Post wall of uterus 7cm. Uterus measured normal and endometrial thickness of 8.2mm. I also had a right ovarian cyst that was removed in the same procedure.

I got married in 2018 and started with Sevista as birth control. In May 2018, small uterine fibroid measuring 11x7mm was discovered. Over the years, I've had several pelvis ultrasounds which found functional ovarian cysts coming and disappearing. In September 2020, I had severe pain in my lower abdomen and upon doing an ultrasound, I found my fibroid had increased to 37x20mm. All this time, I was on Sevista. After that, while my periods were on time, but there was very scanty flow lasting for 1-2 days for the next 3 months (Oct-Dec). In Dec, my doctor put me on Ovral G, after which I got my periods on January 17 2021 and they were heavy and lasted 5 days, with pain in lower abdomen. On February 1 2021, I had another ultrasound that showed bulky uterus (10.2x6.4x5.9cm) and thickened endometrium (14mm), but no mention of fibroids. My doctor told me to continue with Ovral G for another 3-6 cycles and try to get pregnant as soon as possible (according to her, getting pregnant can solve a lot of these issues). I'm not sure of the treatment I'm getting. I have never had bulky uterus or thickened endometrium before, even before myomectomy. My Thyroid and Sugar levels are normal. I have been trying to lose weight for the last 3 months, I am doing yoga, cycling, and have reduced carbs in my diet, but there has been negligible impact on my weight. I want to understand a few things -

1. Is Overal G appropriate for my condition or is it worsening my fibroids?
2. Is it true that pregnancy will solve these issues? My husband and I are going through financial troubles and would not like to have a baby amongst this. I would ideally like to wait for another 2-3 years before getting pregnant. Is it possible for me to wait?
3. I want to explore other options - homeopathy or ayurveda. How effective are these in treating fibroids?
4. How can I lose weight? It seems impossible with this condition.

Brief Answer:
Better not to postpone pregnancy

Detailed Answer:
Hello and welcome to Ask A Doctor service.
I have reviewed your query and here is my advice.

Considering your age and previous surgery for myomectomy and ovarian cyst, I too feel that pregnancy should not be postponed.

I don’t think Ovral G has done anything harm
The uterine size has increased because of fibroid which is not due to tablet.
Endometrial thickness varies from early to late days from period. So that need to be followed up if increases more.
Present situation can be managed by stopping Ovral G after next period and trying for pregnancy.

Meanwhile get your husband's semen analysis.
I am not aware of any useful and reliable treatment in homeo or Ayurveda for fibroids.
It is better to discuss with a dietitian and modify your life style. Some ladies get benefit from Zumba .

Hope my answer helps you

What is the harm in waiting? Why try immediately? What if there are complications in pregnancy due to fibroids? We are not very keen on having a kid in the current situation that we are in.

Brief Answer:
Need more details

Detailed Answer:

Ovarian function will not improve by waiting.Similarly the fibroids might increase in size .If it increases more than 5cm ,you will need surgery again.
Possibility of multiple fibroids again
Hence suggested to plan pregnancy.

So what you are saying is that pregnancy is to avoid any future problems. My question is - how does getting pregnant right now solve the fibroids issue? Will they disappear because of pregnancy? No, right? You guys have no solution for solving the fibroids issue. And what if I have a baby, but fibroids appear again. What will you suggest then? Hysterectomy? I am simply asking you is there a solution to reduce fibroids.

Brief Answer:
I can understand your concern.

Detailed Answer:
Fibroid is usually a benign tumor of uterus.
Pregnancy will not heal or prevent fibroids.
But repeated surgeries can be avoided by becoming pregnant now to reduce complications.

The present fibroid might increase now and may require surgery.

There are few medicines tried for fibroids, but the results are not satisfactory and side effects are more
