Question: Need to know how to retain the frenulum from detachment during circumcision, as it provides the elasticity whenever there poses a problem of scanty urination in summer. A slight jerk by the lower foreskin pulling the frenulum opens up the channel for a easier outflow. Please clarify how to retain the glands at the tip intact..? I m 56 , need for circumcision due to excess growth of foreskin...n diabetes.m

Hi,

Very true retaining the frenulum helps.
Yes, diabetes predisposes for balano-posthitis and needs circumcision one this happens.
Please discuss with your Surgeon about the issue and it will surely help the Surgeon to decide the exact line of excision that removes the extra skin yet retain the frenulum. This is obviously in the hands of the Operating Surgeon.

Till then do the following:
Clean the area with plain water 2 to 4 times a day and and keep it dry with a pure cotton cloth or a dryer.
you may apply liquid Povidone Iodine locally or a Metronidazole cream- nothing else at all. We have seen no results with any other solution, cream or ointment. So do not try anything else; if you are using any, please stop it.

A course of antibiotic with metronidazole orally for 5 to 7 days should help if there is an active infection.
Get your Blood Sugar and other parameters about diabetes checked regularly and keep diabetes under control.
Use loose underwear and pants.
Avoid hot and humid atmosphere.
This may avoid the necessity of circumcision in some cases.
