Question: Hi my son has had a painless lump boney and hard just below the inside right knee on tibua appear played football yest ok. Had xray done ok but sending us to specialist appointment to rule out cancer really worried having to wait, could it be lots things? had bloods taken tday too would that show cancer quicker if there

Could be exostosis.

Hi I am Dr Gopal Goel Orthopedic surgeon.
I have read the details and seen the picture and in my opinion this painless swelling in upper tibia (R) could be an Exostosis- A benign ( non cancerous) bony tumor which , unless it is causing problem need not be operated upon.
The site in picture is amongst the commonest site for this swelling.
This is a common condition and appears at this age only. If it is exostosis then blood tests would be normal.

Even though its non painful?I know you cant say for sure but is it unlikely to be malignant tumor? Osgoodslatus couldn't sit that far over then?
What scans/procedure should we hope to have done next? How will this be confirmed? Will biopsy still be done if diagnosed as ? Thanks just waiting is causing all so much stress!

Hi,

Osgood Schlatter's is a different disease altogether, It is not at the same site.
X rays are generally good enough, but if they are not clear then MRI can be done to define the tumor.

The biopsy is rarely required.

I can understand your stress and that is why I said earlier, do not worry.

Let the reports come at first through.

Hi thank you doctor

We have just had the blood results back as normal would that more than likely show a malignant tumor?, he has noticed bump for about 3 weeks seems ok in himself if not looking a little slender. Would the normal xray have more than likely showed up anything to worry about?

Really appreciate your thoughts so worried.

In my opinion this is a benign ( non malignant ) Swelling.
The blood reports are normal- that is good .
X rays might throw some light on the swelling.
I do not think that there is any cause for worry with these reports.

Hi Dr

Thank you for your support really appreciate it at this difficult time

We seen orthepeodic specialist on thu, they seemed fairly confident was a benign lump possible Osteochondroma exotosis as you mentioned and that the normal xrays were good. Requested an MRI scan which we are having done tues so fingers crossed puts end to our worry.

If this is the case please god is my son ok to continue playing football/sport at minute A friend of our had similar thing removed as played rugby and were worried about injury to area.

Also the complications of this tumor still worrying mentions possibly turning malignant especially with HME Some of the complications that may result from an osteochondroma are:
Pseudoaneurysm (false aneurysm): This is the collection of blood between the two outer layers of an artery.
Claudication: This is a cramping pain, usually in the legs, that may occur after exercise.
Acute ischemia: This is a blockage of the blood supply.
Phlebitis: This is inflammation of a vein.
There’s about a 1 to 6 percent risk that a benign exostosis resulting from HME can become cancerous. When that happens, it’s called an osteosarcoma.

Sorry to be pain can he just not have it removed? Will forward on MRI scan thanks again

At this stage or age ,he is very unlikely to get any one of the complications.
Watch for rapid growth, change in color of skin ,pain or any mechanical block- these are conditions which necessitate removal of tumor. Otherwise removal is a personal choice but a little later in life -say around 17 yrs of age( after skeletal maturity).
The complications can occur in a osteochondroma present in post compartment due to pressure on blood vessels - not likely in your son's case.
At present allow him to carry on with his normal activities.
