Question: Hi, doctor. Recently, the implant I had done several months ago was now ready for an abutment. My dentist gave me options of either the stock abutment (Strauman) or have impressions done for a custom abutment to be made. I went with the stock abutment but now I'm questioning the choice since it was not the exact size millimeter the oral surgeon suggested and it is hurting the gum somewhat. Stock size is 3mm and custom one needed is 3.5mm. My question is this - can u tell me what the differences are between the two, would one be more comfortable than the other, and other pros and cons. Thanks so much.

Hiii first of all thank you for approaching us with your query. I read through it . I must say that stock abutment has fixed sizes. In some cases angulation of the implant in the bone may be slightly tilted so in that case stock abutment wont fit properly so we may have to make custom abutment according the angulation of the implant. Then it will fix properly on the implant. So nothing to worry. Customised abutment will be better if its hurting your gingiva because its not fitting properly onto your implant. Only thing is that customised abutment will be costly compared to stock abutment as it has to made acording to your implant angulation.Hope you understood what I meant. Just one follow up question. When dentist takes this stock abutment off and takes impressions for custom, I know impression then goes to the lab. What is put on implant during the 10 day wait - does healing cap go back on?

Also, when dentist removes this stock abutment which is in there now, should impressions for custom be done immediately (same time) or should it wait a few days?

Healing cap will be put back

Detailed Answer:
Hiii read through your query. For the first query I must say that once impression is made the healing cap is placed back for the labwork to come back usually it takes few days unless he has a lab of his own in his clinic.
For the second query for custom abutment either we can make impression with the stock abutment which will be customised in the lab or if we want the new customised abutment we can make impression that time itself. I usually make impressions that time rather than waiting for few days which is a waste if time for both patient and dentist. Hope ypu understood what I meant .

Just want to clarify something. I have this stock abutment on now for several weeks and going to have dentist take it off and make me a custom one with the parts the oral surgeon gave me.
My question is this - once the stock abutment is removed, wouldn't it be safer to leave the area alone for at least a week for the gum to re-configure and heal before the dentist begins the process of building this custom abutment?

Yes you can leave the area alone but with healing cap.

Hiii first of all thank you for your followup query. I read through it and I must say that you can leave the area but should place healing screw. Otherwise food debris will get accumulated inside the fixture and can later cause infection. Hope you understood what I meant. Thanku and have a healthy day.
