Hi, Doctor. Just A Follow Up On A Question Posed

Question: Hi, doctor. Just a follow up on a question posed to you a few weeks ago where the condition seems to be getting worse involving the throat - feeling of heavy mucous, difficulty swallowing, lump in the throat.
Had all the tests and prescribed many acid reflux meds - none seem to work at all, leaving me to believe this is mostly extreme anxiety and stress as u mentioned. You mentioned possible meds I could try and/or anyiolytics. Could u give me specific names of a few of these to get from my doctor - ideally that would target the nerves in the throat - thanks so much

Brief Answer:
Follow up

Detailed Answer:
Hello,
Thanks for follow up.

In case we assume that it is totally due to anxiety and nothing else can be attributed for it I feel low dose tricyclic antidepressants will be first choice considering your age. Although they have the side effects so they need to be started from a very low dose and dose increment can be considered in gradual manner depending on the response. I generally prefer dothiepin or nortryptiline in similar cases.

Since the class of antidepressants take time, the benzodiazepines can be considered for a short time till the antidepressants show response. For a couple of days/weeks I often prescribe clonazepam 0.25 mg once a day to three times a day.

The above mentioned medications are prescription only medications and need a valid prescription to procure them from pharmacy.

I hope this helps you.
Feel free to write back to me if you have more questions.
Thanks and regards.
