Question: Hello, doctor. Recently, had a fall and as a result have an injury of a stress fracture on left hip. My doctor has said that takes some time to heal and also how pain can be felt both in groin and leg during the healing process. However, I am also very much uncomfortable with the leg on the injured side having an almost constant tingling and pins and needles type feeling. Could this also be a post symptom of having this injury and if so, is there anything I can do to alleviate this discomfort?

Thanks for the query. I can understand your concern. According to your statement, you have been suffering from sensation of constant tingling as well as pins and needles on the leg due to stress fracture.

According to your description, you may suffer from pinched nerve that can be resulted from application of excess pressure by surrounding tissues like muscle, tendon, cartilage or bone to a nerve or set of nerves due to injury or damage.

A pinched nerve usually affects one side of the body and it can be mild to severe. Symptoms of pinched nerve is tingling sensation which may be felt like pins and needles or electric shocks. Injuries, repetitive movements, fracture etc are the reasons of pinched nerve. Electromyography can be helpful in evaluating pinched nerve.

Pinched nerve usually go away over time without any medical treatment. Rest and movement restrictions are the keys to manage pinched nerve. Splinters can be used around the affected area for movement restrictions. Applying hot or cold compresses on the affected area may be helpful too.

NSAIDS like ibuprofen, naproxen or aceclofenac can be taken for pain relief and steroids for reducing inflammation or swelling. In some cases, surgical intervention may be needed to release the trapped nerve.

As your pinched nerve may be resulted from stress fracture, so with the healing of stress fracture, it will be healed too.

Hi, doctor and thank u for your answer. Since it has been almost 2 months since injury occured as well as having tried most of the remedies u suggest and pain remains high, I believe there are injections that may release a pinched nerve since I agree completely with your diagnosis. Would u know what type of injection could be done?

Hi,

Thanks for joining again here. You are absolutely right. Injecting steroid medications into an area of inflammation or the particular site of the pinched nerve may be highly effective for relieving of pain. Local anesthetic like lidocaine can also be added to the steroid shot for rapid relief.

But rest and restriction of movements should be maintained for at least 48 hours after getting the steroid shot. Mild soreness may be felt at the site of steroid injection for 24-48 hours. Just apply ice pack for twenty minutes at a time. Repeat applying ice pack if needed. Avoid applying heat to the painful injection site for 72 hours.

Lumber epidural steroid injection or intralesional steroid injection after detecting the particular area of pinched nerve by MRI can be applied. But steroid injection may take several days to start working. It may take up to two weeks to show its full effectiveness in some cases.

You should consult your doctor promptly to get a prescription for injecting steroid medications into your affected area as steroid shots are only available on prescription.

Hi, doctor and thank u for your excellent advice. My doctor agrees with your assessment and I am scheduled for a steroid injection in one week. Before closing discussion, would like to ask your opinion on a matter separate from the hip injury and one that has been ongoing on and off for some time now. Often, I experience an uncomfortable aching as well as a heaviness type feeling in both legs. I am sure this could be attributed to a number of things but wanted to get your opinion on what this could be and if u could suggest any remedies. I have tried meds such as advil, etc but they do nothing to alleviate the discomfort. Thank u again.

Thanks for joining again. Experiencing uncomfortable aching and heaviness type feeling in both legs occasionally is normal and not a matter for concern. If such type of sensations persist for long or develop additional symptoms like severe pain and swelling, then some investigations should be done to detect the exact underlying causes for your present sufferings and then treat them accordingly.

Again, such type of symptoms may be resulted from poor blood circulation in the legs due to any cause, varicose veins, high cholesterol, high blood pressure, smoking, obesity, overtraining syndrome, restless leg syndrome, spinal canal stenosis and many more.

Some measures such as elevating the legs 6-12 inches above the level of the heart, weight reduction, wearing compression stockings, stop smoking and drinking alcohol, controlling lipid panels and blood sugar strictly, being active, avoiding strenuous workload, salt restrictions etc may help to reduce your present sufferings.

As you occasionally suffer from uncomfortable aching and heaviness type feeling in both legs without any type of swelling or severe painful conditions, so I think there is nothing to worry.

Hi, doctor. Just one last follow up question and thank u so much for all your good advice.
I am somewhat on the fence as to the benefit of getting that steroid shot next week (it won't be anything major, doctor would be doing just one or two trigger shots of I assume cortosone)
The symptoms of the injury on hip have slightly improved but still residual effect. Two part question - would a minor trigger shot like this help at all at calming the nerves in that general lower extremity area and also, any benefit at all to possibly helping this uncomfortable aching in legs? Thank u so much

A steroid can help to reduce inflammation and swelling around the area of particular pinched nerve. In simple words, steroid shot can reduce the inflammation and swelling around the pinched nerve which is causing the shooting pain down your leg. Pinched nerve is caused by undue pressure upon the nerve due to inflammation and swelling around it. Steroid can reduce swelling and thus may help to reduce pressure upon the nerve. On the other hand, steroid can reduce inflammation and inflammation is the ultimate source of pain. So reducing inflammation, can reduce pain.

Steroid shot may need 1 to 4 days to display its full effectiveness and you might be pain free within weeks and within 6-8 weeks, you might be back to normal. Again, some people may notice improvements within few hours after getting the steroid shot.

Usually when other medications and measures are failed to reduce the pain or discomfort due to pinched nerve, then steroid shot is the choice of treatment along with some measures that I have already mentioned.

If steroid shot is failed to subside your present sufferings, then surgical intervention may be needed to release the trapped nerve.

I think steroid shots will work and help you to subside your sufferings gradually. Don't be worried at all before getting the steroid shot. Just remember that treatment are available for pinched nerve and in some cases, it may go away over time without any medical treatment.
