Question: Hello,

I'm looking to know about my syphilis diagnosis. This was a nonreactive RPR result with a positive TPPA result. I was treated for syphilis last year with penicillin - does this mean I am re-infected? Only had sex 5 months prior and have no clear syphilis chancre (I do have a raised wart on my index finger but does not appear to look like a chancre) and has only come up 11 weeks since my last sexual interaction.

I have gone through your query and attached image. Here is my advice.

As per your history and non reactive RPR and positive TPPA test report indicates old treated syphilis. It doesn't indicate reinfection.

The sore on finger may not be syphilis chancre as,

1. Finger is not usual site for primary syphilis chancre.
2. Usually chancre heals within 3 to 6 weeks.
3. It may not develop so late i.e. 11 weeks.

Still to confirm diagnosis I would suggest you to consult with physician for detailed medical examination.

Thank you so much. What test would be recommended to actually confirm if this is not re-infection vs. primary syphilis? Would it be another RPR test? Just curious so I can verify with a physician.
