Question: Hello, I am taking 500 mg depakote ER twice a day for mood stabilization and 20 mg propranolol three times a day for tachycardia. I have been having chest pain for four days that radiates to my neck, jaw, back, and arms. I got it checked out at the ER and all my tests were normal. I am afraid I'm having a coronary artery spasm. Does this sound like a coronary artery spasm to you and would that show up in the troponin level because on Google it said you have to wear an ambulatory monitor or get an angiography of the chest done to diagnose it?

Anyway, I would recommend performing a dobutamine stress echo to help investigate for possible coronary artery disease.

A cardiac stress MRI would be a more accurate test to examine the blood flow in coronary arteries, the presence of possible myocardial ischemia and help examine better the cardiac tissues and structure.
