Question: Hello. I am concerned of ALS. I had experienced random full body muscle twichtes for over a week which have fairly subsided. There was no focal area. I don’t have muscle weakness, but I have noticed my feet have been sweaty for the last couple of days. -Previous Post to you.

Hello Dr. I am more concerned and I can’t get to an neurologist until late this month. I feel more muscle movement in areas after I move them. I’m not sure if it’s heightned anxiety or another symptom. Does this sound like BFS? I’ve lost weight from this anxiety over the last two weeks - 6 lbs because I was unable to sleep for about 10 days and didn’t have much of an appetite from fear. I still do not feel any muscle weakness at all, my coordination is not impaired. I was able to do push-ups and other leg lift exercises today. My calves have heightened twitching after movement, thensettle afterwards. I’m very concerned.

Good morning and I'm very sorry that you have a heightened level of anxiety over this problem but once again, based upon your symptomatology and retention of excellent muscle strength and coordination when doing exercises and other daily activities the likelihood of this being some form of motor neuron disease such as ALS is extremely unlikely.

Your age is also against this diagnosis as it is typically seen in older individuals beginning in the 5th or 6th decade of life but being even more common in its onset in the general population in the 70's. Anxiety, fatigue, poor eating and poor hydration are all things that will definitely make something such as BFS more pronounced.

I appreciate the fact that you've experienced other medical conditions which may very well be at the root of your concerns and fears, however, I feel that your best course of action would be to engage in daily exercises of things such as deep breathing and whole body relaxation in a conducive environment with softer and pleasant music in the background, or even natural sounds if you have access to an isolated outdoors setting where you can clear your mind and direct your thoughts to allow the fasciculations to slow down on a limb by limb basis. This in fact, is one of the techniques that our behavioral therapists will utilize with other folks who have similar concerns over twitching muscles. It is a very powerful way for the body to calm down this excessive activity causing neuronal hyperactivity and eventual muscle twitching.

If you'll focus on this type of daily or even twice daily activity and continue your other routines of work, exercise, etc. then, I believe you will find the problem becoming much more diminished over the course of several weeks. This is what I counsel my patients to do both on this channel of telemedicine as in my practice until they can be seen in the office or until testing has been done.
