Brief Answer:
Some typo error here...

Detailed Answer:
Hi,

It seems there is a typo error in your post. Therefore I am unable to understand what your current problem / situation is. Do you mean your blood pressure is the same despite medication? If this is what you meant, I would like to know since when have you been on this medication.

Awaiting your response...

Brief Answer:
Repeat blood pressures...

Detailed Answer:
Hi,

If he is on telsartan for more than 6 years, then I would not change it based on a single blood pressure reading. At times it is normal to find fluctuation in blood pressure readings. Moreover 154/99 seems to be a reading recorded on automatic home blood pressure monitor where it is normal to find some fluctuations. I will recommend getting BP recorded manually by a health care provider on atleast 3 different occasions. If BP is still high, another antihypertensive medication can be added.

Hope this helps. Let me know if you need clarifications.

Regards
