Question: For rheumataolgist or dr familiar with treating silent psoriastic spondyloarthritist causing intercostal neuralgia

I was diagnosed with this 2 years ago and normally it isn't a big problem, with occasional painkillers sufficient to take care of it. This week the problem has gotten much worse with severe pain in middle of my back through shoulder blades. I was recommended to take Lyrica (pregabalin). What dosage do you recommend, how often per day, and when should it be taken - before or after meals etc?

Hello dear and welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Lyrica (Pregabalin) is a very good alternative to treat intercostal neuralgia.

I suggest you to begin with a dosage of 50 mg three tablets a day, 150 mg/day.

The dose may be increased to 300 mg/day within 1 week based on efficacy of the medication.

You can take Lyrica with or without food.

I would suggest taking it after meals.
