Question: Dr as per your advice i am going try exclusive bf..but there are few scenarios which i would like to have your suggestion about. 1. my daughter sleeps during the feed...no matter what i do she wont get up..in this case should i consider her sleep in 30mins of feed duration? or wait for her to get up and feed again?? 2.what shall i do if the baby still cries out of hunger after 30mins of bf?? please advise.

Thanks for asking back once again. Let me clear your doubts.

You need not wake your baby. Breast feeding should be given on demand. If the child cries out after 30 mins of bf, rest assured that it is not out of hunger. It could be due to a craving for mother's affection. You will have to capre for it accordingly. But do not put the baby again on bf.The milk from that breast would have been exhausted anyway and you need to give it time to fill.

if she sleeps within 5-10mins of bf and wakes up hungry I should not feed?? she cries a lot and no amount of consolation could stop her.. what should I do?

If you have completed the feeding, do not feed further.

Detailed Answer:
If she has not completed her feeding (may take 10-20 mins), you have not exhausted your breastmilk from that breast. In that case, do feed. In case she has received the full feed, please do not feed further even if she cries. Its possibly more out of habit that need.

how to know if the feeding is full..bcos she dozes of with nipple latched in her mouth within first 10mins itself..?? how to tell if the cry s out of hunger or something else..?? and what to in that case..

Depends on after how long she cries out.

Detailed Answer:
If she dozes off within 10 mins of latching, consider the feeding to be more or less complete. Still, if she cries out within 15 mins or so, do put her to breast once again but not for more than 15 mins. If it is after more than 30 mins of that, possibly the crying out is unrelated and more for the mother's touch.

The nature of cry can be differentiated but you need to develop an ear for that. It can be hard. Let us not count on it.

thanks ..will try but if she gets within 15 min ..should i switch the side or latch her on the same side?? also, there were instances when she had sucked for 20-30mins without deep sleep but still craves for milk... in such casex we have given bottle feed only then she used to calm down.. now for exclusive bf what should be done in such situations??

Do not switch to the other breast during a single feed.

No, do not switch sides. Keep the other side reserved for the next feed. Unless you do that, filling would be inadequate. On the other hand, it is pointless to feed beyond 20-30 mins as by then the breast would already be depleted and any further such would just be a waste of energy. Please do not give bottle feed to calm the baby. If the baby is hungry and sucking, the breast milk will automatically increase and her requirement will be met. That will not be the case if you give bottle at the end. The situation might be a bit hard for you but you would need to coax the baby.

doctor I tried this entire day after feeding for liken 10-20min she used to stay calm for another 30mins then it used to become very difficult to calm her down.. she was struggling crying n not able sleep due to empty stomach the entire day.. i switched breasts after every 1.30hrs bcos she used to cry uncontrollably due to hunger...and in the night after struggling for 1hr when she did nt have full stomach and wanted to sleep but was not able to sleep due to empty stomach my parents gave her bottle feed n then only she could sleep.. m scared to try it again today.. please advise what should i do ..

I understand your situation. I think that it is more out of habit and not due to hunger itself. Moreover, switching the breast every 1.5 hours only makes things worse. The feeds are supposed to be given an 2-3 hourly interval and if one breast is used for a particular feed, it is not supposed to be used for the next feed, and only for the feed that comes thereafter. So, effectively that breast gets 4 hours to fill. So, when you are offering it at only 1.5 hours interval, it is actually no wonder if the child's hunger is not met. The rest is a matter of habit.

Things become complicated once bottle feed is introduced as in this case. So, it would be really hard for me to provide a solution.
