Hello dear. Welcome to Ask A Doctor service.

So the ECG is suggestive of right bundle branch block with strain pattern. It is also suggestive of right ventricular hypertrophy. Heart rate is around 75 per minute. Such changes are seen in right heart strain due to disease of lung. A chest x-ray and echo study should be done for further evaluation.
