Brief Answer:
Should visit the hospital as soon as possible

Detailed Answer:
Hello,
Fever, low bp and fatigue are suggestive of infection and also raises the possibility of sepsis. Does he have any cough, shortness of breath, chest pain, burning urination to look for source of infection. So it's important to evaluate him on urgent basis. So he should get some investigations like Hemogram with wbc, esr and LFT RFT.
Also, other investigations for source of infection like urine routine and chest x ray.
Give him some ors or salt sugar water at home to increase the bp and continue to monitor bp. He ll also needs antibiotics for infection.
So overall, it's important to visit the local doctor and ask for investigations.
Hope this helps you and get back if you have any doubts.
