Hello. Thanks for asking from HCM.

I can understand your concern. As your son had 2 episodes of nose bleed, it needs evaluation. First we will find the cause and then treat it

: Direct nose injury - It could be suspected if there is swelling over nose, pain, redness along with clot. You can ask your son about pain and direct injury.

If it is the cause, ask your son not to do nose picking. Avoid any further trauma. You can use saline nasal drops to open nose. You should consult ENT specialist for evaluation of wound, need of antibiotics.

: Head injury - It can nasal bleeding if there is fracture of skull base just above nose roof. It occurs with severe head injury. It presents as headache, nasal bleeding, sometimes leak of brain fluid (CSF) and loss of smell sensation (anosmia).

If his symptoms resemble above features, you should ask him not to blow nose, not to do nose picking, not to bend forward. You must consult doctor as soon as possible. He might advise CT head to find out fracture and defect. Further treatment will depend upon CSF leak. Because rest all symptoms subside and not life threatening.

CSF leak may require medication to stop leak, lumbar drain (drain placed in lower back spine to drain CSF and thus relieve CSF leak) or surgical repair of bony defect. If there is no CSF leak, few medications like painkillers will relieve symptoms.

Hope it will help you. Take care. Thanks.
