Why Is The Iris Cut During A Cataract Surgery?

Hi. My mother is experiencing a complication from cataract surgery. The doctor said her iris tore and now she has a black spot that is causing her to experience an annoying glare. Now the Doctor changed his words by saying he purposely cut the iris. Why would he cut her iris during routine cataract surgery?
