Hello, your symptoms suggest internal hemarroids or anal fissure. First and foremost, you should avoid constipation with increased consumption of fibre rich diet( green leafy vegetables), more fluid intake ( NOT RECOMMENDED for you as you have heart disease), liquid paraffin or cremaffin syrup at night. Sitz bath is also of great help. If symptoms do not improve with these medical treatments, surgery is another option.
Regular BP check up and control of BP is a must for you, because high BP can increase bleading during defecation.
Straining at defecation should always be avoided, because it over burdens your heart.
Hope these advices help you.
