Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Well, first this may be recurrent piles which are possible after surgery and this is the common cause. Second, some other anorectal issues may be present and include anal fissure or rectal polyps.

These are primarily treated with a stool softener, diet, and sitz baths. If there is no improvement in next few weeks, then it would be good to do colonoscopy or anoscopy at least to see what kind of treatment may be appropriate.

Other causes may be more serious and include tumors or inflammatory bowel diseases and these should be ruled out with colonoscopy which must be done if there is low blood count present or rapid weight loss.
But, if this is aggravated by constipation then this is more likely to be the benign cause.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein
