What Is The Right Proportion Of Dexolac To Be Given To A Baby?

Hi, I have 13 days old daughter. I am feeding her with Dexolac special care as she is underweight and is a premature baby. Please let me know how long can we keep the Dexolac preparation ? Also let me know, if we can prepare it in bulk and preserve it in a thermos flask ? Will it be safe for my baby?
