General Surgeon 's  Response
1) your friend may have cirrhosis (liver damage) with portal hypertension (spleen enlarge) associated with portal vein thrombosis.
2) liver transplantation is treatment for this but portal vein thrombosis is relative contraindication for this depending upon severity of thrombosis.
3) his quality of life depends on his current medication status and current investigation details. in case of extended thrombosis, without possibility of transplantation, he need to continue medication support for better life.
