Hello Sir and it is nice to see that you are concerned about your health and want to take a wellness approach at managing your risk factors. You are borderline hypertensive diastolically which is the lower number of 90 in the 130/90. That is a risk factor for heart disease and your doctor would probably recommend the DASH diet to lower your sodium and get on the right nutritional path. As far as your cholesterol being 7.3, that places you at extremely high risk for a heart attack. I suggest a lipid profile but your doctor will probably want to put you on a statin medication right away along with implementing the DASH diet and an exercise program safe for your current cardiac status. Figure that today is the FIRST DAY OF THE REST OF YOUR LIFE AND IT MUST START NOW to attemp to heal your inflamed body. So, 1)Make appt with doctor regarding your cholesterol level and your blood pressure; 2) print out a copy of the DASH diet and start following it immediately; 3) cut out the salt completely, no more table salt and limit it to 1500 - 2300 mg sodium (not salt, sodium); 4) if overweight, set goat to lose wt through cutting calories and exercising. Furthermore, if your sugar was 4.9 fasting you meet the criteria to have Diabetes. Your doctor needs to address controlling your blood sugar as Type II can be controlled by diet and exercise but he may want to start you on an oral hypoglycemic agent until you can get your lifestyle controlled to support a healthy blood sugar. So, my dear, make that appointment and commit to lifestyle change and the fact you probably cannot do it without the help of medication right now.

My best wishes to you. I am here when you have a million other questions after your doctors visit!

Cardiologist Dr. Dr. Meriton Siqeca's  Response
Hello. Thank you for your question and welcome to HCM. I carefully read your query.

A blood pressure of 130/90 mmHg is considered within normal ranges. A cholesterol of 7.3 mmol/L, however, is considered high. Yes, cholesterol is an independent and important risk factor for cardiovascular disease, but there are not parameters that can surely confirm at what age you will get the attack, as it may also never happen. There are various risk scores that calculate the probability you have a heart attack or a stroke in the next certain amount of years. One of them, which you can find online and calculate your risk, is the Framingham risk score. I suggest you to immediately take measures to reduce your weight. This can be achieved by more physical activity, a 60-minutes a day activity such as brisk walking, jogging, hiking or biking, and by removing animal fat from the diet, and cook with olive oil. Other measures that should be taken are: not skipping your meals, especially breakfast, have more vegetables and fruit - up to 300 grams a day, reducing your alcohol intake to a beer a day if you drink, eating the white of an egg and not the red, less red meat and more fish and chicken. If you are over 45 years of age, starting on a cholesterol-lowering agent is advisable. Three months after you have undertaken these measures, you have to run a blood total cholesterol, LDL and HDL fraction and triglycerides levels. If you are going to start a cholesterol-lowering agent, also AST and ALT should be checked.

I hope I was helpful with my asnwer. Best regards.

Dr. Meriton
