What Is The Best Way To Take Care Of A Person Who Is Suffered From Stroke?

Could you help me to know how to support someone with stroke in the scenario below : John is a 76 years old man who has been hospitalized for respite care following a stroke. He has Recovered some speech and mobility. His elderly wife, Mary, has looked after John. Three of their Children live locally but are not in the position to take John into their home. John has confusion at night and sleeps very poorly. He constantly asks to be allowed home and can get extremely agitated. 1) outline a needs assessment on John s holistic requirement to ensure continuty of care 2)Describe how the healthcare assistant could relay information to members of the team in providing specialised services to meet John s needs. 3)Develop an action plan that would deal with John s poor sleep and night time confusion 4)Identify the precautionary measures that should be taken to ensure the safety and well being John 5)outline the relevant information necessary for discharge plan.

CVA PATIENTS R TO BE DEALT WITH EXTREMLE PATIENTLY, CONFUSION N SLEEP PROBLEMS NOT TO BE DEALT AGGRESSIVELY, CAN START TAB EPIVAL OR RIVTRIL DROPS FOR CONFUSION N RESTLESSNESS, BATHROOM VISITS REQUIRE EXTRA ATTENTION, CATHETERIZATION IS AN OPTION,CATHETEIZED PATIENTS PRONE TO INFECTIONS SO GIVE PROPER ANTIBIOTIC COVER, DISCHARGE PLAN WOULD INCLUDE THE ABOVE PLUS MEDS DEPEND UPON WHETHER THE STROKE WAS ISCHEMIC OR HEMORRHAGIC. IF HEMM THAN BP MEDS ONLY, IF ISCHEM THAN ASPIRIN 75 , OR CLOPIDOGREL 75 PLUS ATORVASTATIN IN ADDITION TO BP MEDS. THANX HOPE THAT HELPS
