What Is The Best Treatment For ADHD?

Dear Dr.,

My second son Name : Rithesh (Date of Birth 06.12.2004) was diagnosed Attention Deficit Hyperactivity Disorder (ADHD )at the age of 3. Though he was in special school for one year (last academic year), as advised by the special school (Sakalp, Chennai) he now put in normal school (Roots to wings, Chennai - for UKG) , where there are less students and taught in play way method (Montessori).

To our surprise / satisfaction, after his 5th birthday (last year) we noticed some special skills in him i.e. with some morally support (by holding his hand at wrist) he can write English, Hindi, Malayalam, Tamil (alphabets, words and even answer the questions we ask), do mathematics (addition, subtraction, multiplication & division) without anybody teaching him these skills before. This was also witnessed by his special school teachers & occupational therapist. But his level of interest is very low on these skills.

Though he doesn’t speak frequently, he expresses his need only by questioning (example - do you want water? etc.,) in our regional language (Malayalam), he even sing tamil songs - but not clearly, when he is alone started talking to himself, used to jump and roam around.

So far he is not on any medication as advised by his physiatrists.

There is no any major complaint about him in his present school (normal) but we were informed he doesn’t concentrate in class or the activities given to him. Also his level of cooperation for Occupational Therapy is also very low

Doctor, your most valued suggestions / advises to improve Rithesh’s capability will really be an emotional and great help to us.

Regards,

Ravikumar

Pediatrician 's  Response
Hi, thanks for asking.
I can understand your concerns.
First of all I would like to ask you few questions about him.
Does he use language to express or relate his experiences?
How is his socialization with the peer group?
If both of this are problematic then it is quiet likely that your child may fall in the autism spectrum with savant qualities.
I would suggest you see a developmental and behavioural paediatrician for the formal evaluation to rule out autism.
He may benefit with medications for hyperactivity like the methylphenidate.
He will also benefit with play therapy and occupational therapy.
It will be much better if you structure his day with visual cues. Once his day is structured he will do the activities which promote his savant skills.
I hope I have helped you.
If you have any further queries please feel to ask.
Thank you
Dr. Meera Developmental and Behavioural Paediatrician.
