What Is The Average Dosage Of Keppra For Patients With Seizures?

I am a Hospice RN and have a patient who is taking 750mg BID for seizure precaution. He s had 2 seizures on this dose. I looked up the half life of Keppra and scheduled the meds accordingly, which help in the interium. What is the max dose of keppra for seizure precation? Thank you

Hello and thanks for writing.

The maximum recommended dose for a sustained release preparation of keppra is 300 mg once a day. The dose has to be started at 1000 mg and gradually uptitrated at a rate of 100o mg increment ever 2 weeks till maximum recommended dosage is reached.
Also the dosage in elderly patient depends on their creatinine clearance. SO i suggest you measure their creatinine clearance first. If it is above 80 then full dose can be started, if 50 to 80 mL/min, give 1000 to 2000 mg every 24 hours, if it is 30 to 50 mL/min, give 500 to 1500 mg every 24 hours and if creatinine clearance is less than 30 mL/min, give 500 to 1000 mg every 24 hours.
