What Is My Ultrasound Result Suggestive Of?

Dear kind dr. I am 73 yrs , female. I accidentally found a lump about one finger long and two fingers wide at the right inguinal area without any discomfort except I pull my leg touch my chest. The lump won t show if I stand up. The ultra sound showed Bilateral inguinal hernias, about 4 cm, larger in the right. Both reducible throughout the exam. My questions: 1. Should I wait for more symptoms to have it operated, i am afraid of getting long term post op pain? 2. If I have to go for surgery now, what type of surgery, incision or laparospic, (by the way, I had laparoscopic done once about 15 years ago to remove ovary cyst)? 3. Should I have both sides done, or just right side if it is done by laparoscopic? (No lump on left side, it won t bother me at all.) 4. What is the % to get long term post operation pain? 5. Which type of surgery is better to prevent nerve damage, blood clot, infection , post op pain etc...? 6. Is that ok if I don t use mesh which can prevent recurrence but more chance to hurt my nerve to cause long term post op pain? Thanks so much! Best regards, Helen

Thanks for posting in HCM.
I understand your concern.
Based on the ultrasound scan report, it is clear that you are having 'inguinal hernia' on both sides, which seems to be larger on the right side.
You need not wait for more symptoms to appear and should plan electively to get operated at the earliest.
Hernia repair operation can be performed by means of incision or by means of laproscope. You need to undergo operation on both sides, preferably in one sitting itself.
Post-operative recovery is good and there is very less chances of recovery.
Laproscopic surgery could be preferred as its recovery is faster and with minimal scaring.
You need to take care by not lifting heavy weights and use supportive abdominal mesh until you undergo surgery and recover following surgery.
Kindly consult Surgeon in this regard for clinical evaluation and appropriate management.
