Hi,

Per se, there is no relation between the renal calculus and fluid in the pouch of Douglas for sure.
Both are different problems.

The cause of fluid in the pouch of Douglas can be PID that is a pelvic inflammatory disease, tuberculosis or any other inflammatory disease going on in the abdominal cavity. Since you have given no other history it is difficult to say whether this is just a normal occurrence or due to some disease.
The 4 mm calculus in the renal pelvis can cause severe pain while passing through the ureter or urethra at a later time.
Plenty of oral fluids to have plenty of forced urination can push out the stone out of the body.

Consult your Doctor to get n appropriate management for both the conditions, particularly if there is a cause for fluid in the pouch of Douglas.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant, General Surgeon
