What Does This Nuclear Test Result Of Heart Signify?

Im a 45 yr old female after years of heart problems with no known cause 2 yr ago I underwent a cardiac cath and was diagnosed with a myocardial bridge and 40%LAD . Recently I ad another nuclear stress test done due to continued pains and problems . Test results say small in size , mild in severity anterior apical defect consistent with mild ischemia. TID of 1.2 which is borderline abnormal. Compared with SPECT Myoview from 2 yr before there is now an obvious defect in the apical region albeit mild . another page says abnormal Lexiscan Myoview , 1mm ST depression with T wave inversion , rare premature ventricular contractions. Please help me understand what all this means and how serious I should take this

Cardiologist 's  Response
Hello! Thank you for your question on HCM! Regarding your concern seems that one of your coronary arteries (Left Anterior Descending) shows a dynamic narrowing as a result of the surrounding myocardial contraction. This coronary bridging imply a myocardial ischemia in the territory supplied by this artery distal to the narrowing (as imagine nuclear test reveals).
As you experience chest pain and the performed tests show small anterior-apical ischemic myocardial region; there exists a potential risk for irreversible heart muscle damage in the ischemic territory, and a probability for generating life-threatening ventricular arrhythmia.
I recommend you to discuss with your cardiologist for instituting an appropriate medical treatment comprising negative inotropes. If complains persist then an alternative revascularisation option may be considered.
Hope to have been helpful to you.
Greetings! Dr. Iliri
