Hello
Thanks for writing to HCM

Though heart is full of blood all the time the heart muscle is supplied by three major blood vessels on its surface one on right side and two on left side if there is any blockage in these blood vessels your heart muscle will be devoid of nourishment and you will have cardiac attack
In your brother case all the three blood vessels nourishing the heart are blocked
As he is diabetic with triple vessel disease i would suggest for a bypass surgery rather than stenting

Discuss this with your doctor and then decide.
Hope i have answered your query.
