welcome to Health care magic.
1.Your report indicate that the disc which is seen in between the vertebrae has slipped / moved posteriorly.
2.This can be seen in age related degenerative changes / systemic causes and in trauma.
3.Posteriorly the disc at level C4-5, C5-6 causing mild mass effect on the nerve and chord ( which causes the symptoms in you - like pain, numbness, tingling and so on).
4.This can be treated with simple exercises to neck support collar, and depending on the severity - intervention / operative procedure might be needed.
5.Consult ortho / neurology with this report for further evaluation / treatment.
Hope it helps you. Wish you a good health.
