symptoms of head pressure w/migraine, then eposide of near syncope. Labs WNL. CT HEAD: Bilateral frontoparietal white maatter lucencies most commonly seen with chronic microvascular ischemic demyelination change. Should this person be referred to a neurologist?

There might be some underlying disease or condition which might be responsible for microvascular blockage like atherosclerosis, Hypertension, Hypercoagulable states, etc. Age is also an important prognostic factor in this patient. Yes, you should consult a neurologist for this.
