The severity of shortness of breath with shrinkage changes in brain (likely age related) indicates cardiac compensation most likely. It is the clinical evaluation with examination and supportive data of X-ray chest, 2 D echo heart and EKG will reveal best diagnosis out of various possibilities.
Following signs are consistent with cardiac failure or over exertion from various reasons: - Rales and crackles both lower lobes of lungs - Pedal edema - Jugular venous distension - S3 or S 4 on cardiac auscultation - Hepatomegaly (congestive) in right sided heart failure - Tachypnea - Orthopnea
Take care. Hope I have answered your question. Let me know if I can assist you further.
