General Surgeon 's  Response
Hello,

The severity of shortness of breath with shrinkage changes in brain (likely age related) indicates cardiac compensation most likely. It is the clinical evaluation with examination and supportive data of X-ray chest, 2 D echo heart and EKG will reveal best diagnosis out of various possibilities.

Following signs are consistent with cardiac failure or over exertion from various reasons:
- Rales and crackles both lower lobes of lungs
- Pedal edema
- Jugular venous distension
- S3 or S 4 on cardiac auscultation
- Hepatomegaly (congestive) in right sided heart failure
- Tachypnea
- Orthopnea

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
